Registration open to anyone living in the Oldham area.
Step 1 – Come to Reception
Bring with you:
- your medical card, if you have one
- your NHS number if you do not have a medical card (you can get this from your current doctor).
- Proof that you live in the Borough of Oldham (a recent utility bill, rent book, or other official bill, NOT mobile phone).
Reception will give you a form and a questionnaire about your health which you will need to fill in.
You will also be given a bottle for you to provide a urine sample at your first appointment.
Reception will then book your registration appointment.
Step 2 – Come to your first Appointment
Bring with you the:
- completed forms and urine sample
- proof of any medication you are on (for example, a repeat prescription form or the medication box/packaging)
- Make sure you tell us about any illness or operations that you have had.
If you are over 40 years old you will be asked to have a blood test during your appointment. In the 12 hours before the blood test you are asked not to eat or chew anything. You should drink water and take any medication that you have been prescribed.
Step 3 – Acceptance
If you are accepted as a patient at the centre we will then ask for your notes to be transferred to us from your previous doctor.
